Each of the programs in the seven states, Colorado, Florida, Kentucky, New England, New York, Utah, and Virginia, have reached various stages of implementation.
The experiences of each of the programs thus far have further demonstrated the CHI PACC® model's flexibility through diverse approaches to implementation strategies. Each state involved in CHI PACC® has provided information about their program and services offered in their state.
