HOW TO BECOME A CHI PACC® DEMONSTRATION
Any program wanting to address comprehensive medical needs of children with life-threatening conditions and their families is capable of applying. For example: hospice programs, home care programs, children's hospitals, state and regional "partnerships", etc.
- An existing adult hospice program could hire pediatric staff, and the CHI PACC® proposal could cover costs not currently covered by Medicaid for respite and bereavement care.
- A children's hospital that serves children from a multi-state area might expand their current hospice program to include children from the time of diagnosis, and have improved access to Medicaid reimbursement on a regional basis.
- A state hospice network might develop a collaborative program bringing together and overseeing care through hospice(s), home care program(s), children's hospital(s), and related institutions statewide.
