DEVELOP A CHI PACC® PROGRAM

TECHNICAL ASSISTANCE SERVICES FOR CHI PACC®

The CHI PACC® model was devised to permit flexibility in implementation, and to allow any appropriate entity, from government offices through community-based providers, to tailor CHI PACC® development to accommodate local needs and goals. Children's Hospice International maintains technical support teams to assist state government entities, provider associations, and community-based providers that wish to explore and/or implement the CHI PACC® model.

Technical Assistance Services Available from CHI

For State Government Entities, assistance with:

• Undertaking a state-wide needs assessment
• Developing a CHI PACC® model appropriate for existing infrastructures
• Preparation of a Medicaid waiver application or research and demonstration proposal
• Designing a cost/benefit analysis
• Identifying state legal/regulatory/licensing barriers and potential solutions
• Interfacing with federal regulators
• Ensuring that the State model, as implemented, will comply with the
  CHI PACC® Standards of Care
• Developing a plan for evaluating the program

For State or Regional Provider Associations, assistance with:

• Developing provider CHI PACC® models and education materials
• Identifying funding resources
• Facilitating provider collaborations
• Ensuring that models and materials comply with the CHI PACC® Standards of Care
• Planning support for developing CHI PACC® programs
• Guidance in identifying and dealing with federal, state, and local legal/regulatory issues
• Developing a plan for evaluating programs

For Community-Based Providers, assistance with:

• Establishing advisory councils
• Establishing and conducting focus groups
• Conducting a needs assessment
• Identifying clinical goals
• Managing local collaborations
• Organizational support
• Expansion of services
• Identifying complementary providers for new services
• Contract development with third-party payers

CHI provides CHI PACC® technical assistance through several formats and options:

• Direct consultations on site with members of a CHI Technical Assistance Team
• Consultations at the CHI office with members of the CHI Technical Assistance Team and, if helpful, CMS staff
• Educational conferences with CHI PACC® providers
• Workshops and forums with CHI PACC® providers
